During these hours our business office and nursing staff will be available to answer your calls. Medical questions should be directed to our nursing staff and appointment calls or questions should be directed to our business office personnel.
We have physicians on call 24 hours a day should you need assistance after normal business hours.
Office visits are by appointment except for emergencies. If you are unable to keep your appointment, please call ahead of time to let us know. Our receptionist will be happy to help you reschedule your visit.
Insurance & Billing
Our patient billing and insurance team will gladly discuss any questions you might have concerning physician billing. Our office participates in most types of insurance programs. We will be happy to assist you in the completion and filing of your insurance claims. Deductibles, co-insurance, and non-covered charges are requested at the time of service. Please inform our office if you have a change of name, address, insurance or employment.
For new and established patients, bring the following items with you on the day of your visit:
- A list of your current medications
- Your preferred pharmacy name, address, phone and fax
- Referral number or forms
- Your insurance card(s)
- Picture ID
- A list of questions or concerns you may have
